DEATH CERTIFICATES:
Certified copies of the Death Certificate may be ordered from Dunes Memorial Chapel by calling 541‑271-2822, or from the county health department in which the death occurred. The Douglas County Health Department phone number is 541-440-3513. Coos County is 541 756-2020, and the Lane County number is 541 682-4045. Certified copies are needed for: insurance, insured loans, stocks or bonds in joint ownership, real property in joint ownership, etc. The state of Oregon charges $25.00 for the first copy, $20.00 for each additional copy ordered at one time, and a one time filing fee of $20.00.

SOCIAL SECURITY:
Dunes Memorial Chapel will file form SSA-721 for you. It applies for the Lump Sum Death Benefit of $255.00. There must be a surviving spouse to be eligible. Surviving spouse and/or children under the age of 16 should be eligible for benefits. You must also contact the local office at 3661 Broadway, Coos Bay, phone 1-800-772-1213.


INSURANCE:
Notify other insurance companies or local agents on policies held on the deceased.  Check coverage of home, at place of employment, auto medical coverage, union, health and accident, life insurance, credit accounts such as Wards and Sears which might have death coverage, bank accounts, credit unions, etc. Contact the Veterans Service Officer for G.I. Insurance, 1‑800‑669‑8477.


OFFICE OF FEDERAL GROUP LIFE INSURANCE:
Call 1-800-633-4542 and ask for Form FE6 Claim for Death Benefits.  Their address is 4 East 24th St. New York, NY  10001.


VETERAN'S BENEFITS:
Dunes Memorial Chapel can file for any burial benefits for which you may be entitled.  They will need a copy of the discharge and/or Form DD214.  Upon request, we can secure a flag and make application for a headstone. Additional benefits may be available if the death occurred in a VA facility or if the death was service connected.  Phone the Veteran's Service Officer at 700 Summer St. NE, Salem, OR 97310, 1‑800‑692‑9666 to inquire about any benefits.  To inquire about property tax  exemptions, call 1-800-356-4222. Contact the Veterans Service Officer for G.I. Insurance, 1‑800‑669‑8477.


COUNTY TAXES
The veterans surviving spouse may be eligible to have the property taxes on the residence reduced. Contact the county assessor’s office and provide the military information.


BANKS:
Notify the bank of the death.  Check for insurance on bank loans and installment contracts.  Change the name on bank accounts, government bonds, and saving certificates.  Check the Safety Deposit Box.  It's probably smart to keep the deceased's name on one joint checking account for a couple of months.


HOME AND REAL PROPERTY:
Take a certified copy of the death certificate to either the Douglas County Office of Deeds and Records, The Coos County Office of Deeds and Records or the Lane County Office of Deeds and Documents depending on which county your property is in. All are located in the appropriate County Courthouse.


OREGON STATE HOME LOAN MORTGAGE
To activate the mortgage cancellation, a Certified copy of the Death certificate should be sent to either the county’s Veterans office or the local Oregon State loan office.  The State Veterans Affairs Department can be reached at 1-800-633-6826 or 1-503-373-2373.  Their address is 700 Summer St. NE, Salem OR  97301.


DEPARTMENT OF MOTOR VEHICLES:
 A Certified copy of the Death certificate is NOT required to change titles.  Make a copy of the death notice from the newspaper or ask your funeral director to make a copy of the notice for each vehicle.  Take this and the certificate of title to the DMV Driver and Motor Vehicle Services  1155 S 5th St. Coos Bay


OREGON DEPARTMENT OF REVENUE:
Contact them regarding Inheritance Taxes.  The local office is located at 3030 Broadway, Coos Bay.  The phone number of the Salem office is 1‑503‑378-4988.


INTERNAL REVENUE SERVICE:
The local office is located on 300 Country Club Road, Eugene 97401. The phone number is 1‑800‑829-1040.  The problem resolution office is open from 8 AM - 11:30 AM and from 12:30 PM - 4:30 PM.  Blank forms can also be ordered by calling this number, or for blank forms only, call 1-800-829-3676.


STOCKS & BONDS:
Write to the stock company about the procedures necessary to transfer the stock to your name, or you can contact your local broker for help.  You must have written consent from the State Department of Revenue to sell or transfer CORPORATE stock owned by the deceased. (Form IT‑17)


PUBLIC EMPLOYEES RETIREMENT SYSTEM:
If deceased is a member of P.E.R.S. (public or civil service employee), call the Public Employees Retirement System 1-888-320-7377


U.S. CIVIL SERVICE:
Write to the Bureau of Retirement Insurance and Occupational Health, Washington, D.C. 20415, and ask for Form 2800, the Application for Death Benefits.
